Promed’s home health services include at-home physician and nurse services, lab sample collection, pharmacy door delivery and home physiotherapy. We ensure the best quality and strive to improve overall care and experience of our patients.

Home health services are designed for elderly patients, patients who have mobility or transportation issues, patients who need skilled nursing or rehabilitation or patients who were recently discharged from a hospital. We can provide customized home health care or help caregivers who are overwhelmed with taking care of a loved one.

Services include:

  • Ongoing primary medical care and management of chronic illness
  • Acute care visits
  • Follow up home care after a recent hospitalisation or outpatient procedure
  • Prescription refills
  • Blood work and lab sample collection
  • Physiotherapy and rehabilitation services

Studies have shown that patients who receive medical care at home function better and have a lower rate of readmission.
